Brown Primary Care Transformation Initiative

The Brown Primary Care Transformation Initiative is a project supported by a 5-year grant from the Health Resources and Services Administration (HRSA) that focuses on implementation of the Patient Centered Medical home (PCMH) model, on comprehensive evaluation of the process and outcomes, and on training the next generation of health care providers in the PCMH approach to health care delivery.
Now nearing the end it's third year, the project has assembled a skilled team of coaches and researchers who work closely with other state-level PCMH initiatives, and who assist eight practices that include residency clinics, community health centers, private practices and a college health service. The Initiative’s mission is to promote healthy patients and healthy practices by supporting innovation and sustainable quality improvement infrastructure in primary care.
