        • In the "add forms suite," add tobacco counseling component
        • In the "add forms suite," add the adult hx component
        • In an "FCC adult extended visit," enter information in the health risk assessment component
      • Checking the “smoking cessation advised” box on the MHRI A/P Drop Down component

    • Medications
      • Neighborhood Health Plan (Rhody Partners)
        • Formulary medications preferred: nicotine patches, nicotine gum, and buproprion SR (Zyban®)
        • Completion of Prior Authorization for varenicline (Chantix ®): requires smoking cessation program: http://www.nhpri.org
      • United HealthCare of RI (Rhody Partners)
        • Nicotine patches and varenicline quantities limited
      • Medicaid RI
        • Nicotine patches, buproprion SR/XL (Zyban ®), and varenicline (Chantix®) covered with a Prior Authorization

        • Go to Orders
        • Go to Categories
        • Click under categories: psychology
        • Then click psychological counseling
        • Put in diagnosis smoking or tobacco abuse
        • Click on enter
        • Go to order details
        • Go to instructions and enter .qw
        • Fill in the information that is asked within the quicktext that appears.  Make sure that the patient agrees to be contacted and x the box
        • Under referral to, click the 3 dots and enter quit in the search field
